Otoplasty, or ear reshaping surgery is a cosmetic ear procedure to reshape, reposition, pin, and/or resize the ears. This type of specialised surgery is popular with patients who are unhappy with either the way their ears protrude, or maybe just the general shape of the ear due to birth or following an injury. Once your ears have reached their full size, this type of procedure is safe to go ahead with. Ear reshaping surgery can have a huge impact on the way people feel about their appearance, improving their self-esteem. Otoplasty procedures include:
- Ear pinning: addresses protruding ears, bringing ears closer to the head by repositioning the cartilage
- Ear resizing (ear reduction): reduces size of overly large/disproportionate ears with removal of tissue, whilst maintaining natural

What Happens During an Otoplasty Consultation?
During the initial consultation we will review your medical history to ensure there are no factors that may increase risk during the otoplasty procedure. We will then carry out a physical examination to assess the placement, size, shape and symmetry of your ears. If necessary, we may take photographs of your ears for your medical records. Most importantly we will explain the procedure in full and discuss your expectations accordingly. We will cover all topics relating to the treatment including local anaesthetic, general anaesthetic and sedation to ensure you are completely comfortable before going ahead.
What Does an Otoplasty Procedure Involve?
An Otoplasty is surprisingly complex. The appearance and prominence of the ear is governed by a delicate balance among the curvatures in the cartilage of the ear. Changing the shape of one area, therefore, causes changes in the shape of surrounding areas. To be successful, each manoeuvre must compliment each other. These include such things as cartilage scoring, excision, and cartilage stitches. A careful interplay of these techniques should produce the desired result. Since everyone is unique, the exact surgical plan cannot be determined until a full examination of the ear is completed.

- Otoplasty surgery begins with an incision just behind the ear, close to where the ear joins the head.
- The surgeon will then remove cartilage and skin and sculpt the ear to the desired look.
- Cartilage is then pinned back with sutures.
- In other instances, the surgeon will not remove any cartilage but use stitches to hold the cartilage permanently in place.
- After sculpting the cartilage to the right shape, the surgeon will apply sutures.
Recovering from Otoplasty Surgery
Most patients recover fully within 2-6 weeks after otoplasty, though this can vary by individual. Swelling, itching, and numbness are common in the first few days or weeks, so proper post-surgical care is essential to a smooth recovery.
During recovery, you’ll need to wear a supportive ear band to protect your ears and aid the healing process. It is also important to avoid strenuous activities like jogging, cycling, swimming, and weightlifting until your surgeon gives you the all-clear.

Ear Pinning Surgery
Ear pinning can be completed under general or local anaesthetic depending on the complexity of the procedure and can typically take about one hour.
- Firstly, your surgeon will make an incision behind the ear to weaken or remove the skin or cartilage that’s making the ear protrude.
- Your ear will then be repositioned to the desired place and sewn into place with permanent stitches.
- This process will then be repeated on the other ear. Even if only one ear protrudes, this will make them both symmetrical.
- The surgeon will place a dressing on the head to support your ears.
Ear Resizing
Ear resizing procedures are typically performed under local anaesthetic depending on the complexity of the surgery.
- Your surgeon will begin by making a small incision behind the ear, accessing the cartilage and skin needing to be resized.
- Excess skin and cartilage are then carefully removed. Cartilage may be trimmed or shaved.
- Once reduced, your surgeon may then shape and secure the ear in a more balanced position if necessary, ensuring the ears maintain a natural appearance.
- Incisions will then be closed with stitches, and the ears will be bandaged for support and to aid healing.
Ear Reshaping
Ear reshaping is typically completed under a general anaesthetic but can also be done under a local anaesthetic. Your surgeon will inform you on what anaesthetic is best for your procedure. This type of procedure usually takes between one to two hours depending on the goal you want to achieve.
- Your surgeon will make the incisions behind your ear, then reshape or reduce the cartilage as needed.
- Any excess skin will be removed to refine the ear’s shape and achieve your desired look.
- This process will then be repeated on the other ear to make both ears symmetrical.
- Your surgeon will place a protective dressing on the head to support your ears.
Following otoplasty, large dressings are applied to the ears normally until after your first postoperative appointment. Most patients experience some mild discomfort. If you are accustomed to sleeping on your side, your sleep patterns may be disrupted because you cannot put any pressure on the ear areas. Headbands are sometimes recommended to hold the ears in the desired position following surgery.

What are the Risks of Otoplasty Surgery?
Like any surgical procedure, otoplasty surgeries come with potential risks. These may include:
- Scarring – While incisions are typically placed behind the ear, some scarring may still occur
- Infection – There is a potential risk of infection which may require antibiotics or further treatment
- Numbness – Temporary numbness can occur, and though rare permanent numbness can occur if nerves are disrupted
- Bleeding – There is a risk of excessive bleeding or a hematoma after surgery.
